Portfolio Documentation

Please provide documentation of your experience coaching the following NH MTSS-B practices. Provide documentation for all practices for which you have experience. Documentation can include a completed tool or template, meeting notes/minutes, copies of MOUs/collaborative agreements, or in the absence of another type of documentation, a brief narrative description. Practices marked as required must be completed in order to obtain MTSS-B Level 1 Coaching Certification. You may submit documentation for any additional practices to work toward higher levels of certification. All documentation and narrative responses should be redacted of individual's names or other identifying information.

Assess MTSS-B fidelity with the NH MTSS-B Fidelity Inventory (NH MFI): Schoolwide/Tier 1 and Tier 2/3 modules*

Develop an annual action plan using the School MTSS-B Action Planning Tool (S-MAPT)*

Develop schoolwide behavior expectations and acknowledgment and response plan

Select and implement a universal social-emotional learning (SEL) curriculum

Design and implement a universal social-emotional screening process (including tool selection)

Implement a schoolwide request for assistance process to guide referrals to social/emotional/behavioral health supports and services

Develop facilitated referral pathways to partnering community mental health agencies

Select an array of evidence-informed Tier 2/3 behavioral health practices*

Design a plan for ongoing progress monitoring
